Dear Credit Bureau,

Reference to (date) which you signed the return receipt of my letter sent by registered mail, I hereby send this formal notice that you have not replied to my complaint letter of (date).  

It is clear that I am maintaining strict documentation of my correspondence with you and have observed that it has been over 30 (Thirty) days since you received my letter which is not in compliance with the FCRA that requires you to have responded within 30 (Thirty) days.

I understand if my initial letter was not given attention or misplaced as a result of the bulk of requests you constantly receive. In that case, I have included a copy of my initial request, a copy of the signed receipt of the letter and also the supporting documents confirming the erroneous item on my credit report.
